Advance decisions are legally binding documents allowing you to retain your legal right to decline specific treatment. They must be signed and witnessed to have legally binding effect, and it is recommended that you deposit a copy of the document with your GP, local hospital, and a friend or relative if appropriate.
Advance statements set out your preferences for treatment should you no longer have the capacity to make your own decisions. This document is not legally binding, but may help health professionals to understand your preferences.
The document available through the link below is a simple Advance decision and statement form. While there are commercial companies available who can assist in drafting more complicated documents, if you intend to create the document yourself we recommend keeping it as simple as possible.
